According to the Case Management Society of America, case management is "a collaborative process of assessing, planning, facilitating, coordinating care, evaluating and advocating for options and services to meet needs of an individual and meet the family's overall health needs through communication and available resources to promote quality, cost-effective outcomes" (Case Management Society of America [CMSA], 2010).
